Commissioned by NHS England and NHS Improvement, we led the Medicines Optimisation in Care Homes (MOCH) pilot across The Black Country and West Birmingham, integrating pharmacists and technicians into care homes to support patients and staff. Our multidisciplinary approach included structured medication reviews, waste audits, and system reviews to enhance medicines safety.
This programme has not only improved patient outcomes but also reduced pressure on GPs and A&E services, saving over £149,000 in drug costs and freeing up 62 GP appointments monthly. The care home system review identified areas for improvement, with focused training helping to enhance practices and communication.
