We partnered with Wirral CCG to implement a 12-month integrated medicines optimisation project, targeting high-risk patients to reduce unplanned hospital admissions and improve medication management. By working with community integrated teams and conducting medication reviews, we achieved an annualised cost saving of £159,925 and avoided 118 potential hospital admissions. The project demonstrated a return on investment of more than 7:1, delivering significant cost improvements for the Wirral healthcare system.
